New Patients
Preparing for your Appointment
New patient paperwork is completed electronically prior to your appointment. You should receive your intake forms by email approximately one week before your visit.
Completing your paperwork in advance is extremely helpful to both our clinical and scheduling teams. Your answers allow our team to review your history, symptoms, medications, prior treatment, and imaging before your appointment so we can make the most of your visit and better understand your condition ahead of time.
If you have not received your paperwork and your appointment is less than five days away, please contact our office and we will be happy to resend the information.
Imaging & Medical Records
Please bring or upload any recent MRI scans, CT scans, x-rays, and previous medical records related to your spine condition or treatment.
Whenever possible, we strongly encourage patients to bring MRI imaging on a disc to their appointment. Having the actual images available allows our team to upload them directly into our system so Dr. Crane can personally review the imaging during your visit. This is often much more helpful than relying on written reports alone and can make your appointment significantly more productive and efficient.
If you are unsure what imaging or records may be helpful, our office is always happy to guide you.
